The Emergence of Private Sector Geriatric Care Management

Geriatric Care Management, a multidisciplinary profession made up primarily of nurses & social workers, first emerged as a professional field about 15 years ago when experienced clinical professionals, accross the country began, to leave traditional third party settings. Disillusioned with the large caseloads typical in non-profit and publicly funded agencies, and the limitations of third party payors such as Medicare, a group of about 100 practitioners in 22 states had begun to set up private consulting practices to help family caregivers of frail elders. They met to share ideas and to discuss how they conducted their private clinical gerontology practices. The result of these early meetings led to the formation of the National Association of Professional Geriatric Care Managers (NAPGCM). NAPGCM currently consists of more than 1500 professional geriatric care managers nationwide with a vast majority in individual or group practices. The national association exists to foster the profession of GCM through marketing and public relations, legislative activism and professional development. NAPGCM facilitates a yearly national conference, publishes a practice journal and several other publications. Additionally, individual state and regional chapters host regular chapter meetings and some also hold chapter conferences.

A computer game that may assist in the eradication of diseases and the obliteration of some!

Given that I have thrown this idea out into the world in the hope that it will reach critical mass, it will probably now have to come from some public sector boffins rather than some business engine. At the very least this little essay is a goofy neuro finger print into the mindset of misplaced potential, at best…it’s a viable idea! That is not to say that there cannot be many variations, mutations and developments from this idea – I hope there are. Besides, I no longer care as I’m flying off with aliens – N’yeh!

Psychosoma is an adventure that takes places inside the human body. Not just any human body but the body of the player. Its primary purpose is to assist the player in the healing and repair of any mechanical damage or disease. This is done by creating a reproduction of a part of a brain or body from an actual cat/bio scan which maps the area local to the disease or damage and provides a complete cross-section 3d photograph. That image is then uploaded onto a computer or onto a platform like The Sony PS3 and when integrated with the software, once again becomes a fully fluid representation of the body at the moment of scanning, complete with injury and/or disease.

There are many cases where psychosomatic input can have a significant, if not, profound effects in assisting the eradication of something like a tumor or the time and effectiveness of injury repair through conventional means. There are a sufficient number of cases where there is a total recovery. In such cases, however, it is helpful if the person is able to make massive lifestyle changes, diet, environment or more rarely, a massive a paradigm shift.

Employment opportunities for physical therapist, assistants and aides are numerous in today’s challenging job market. Medical and scientific research has created the need for many rehabilitation services. Our population is growing older at a rate that will require the support of many health care personnel. These are just a few of the reasons that make the choice to pursue a career as a physical therapy assistant or aide a good decision.

Assistants work directly under the supervision of a physical therapists. The majority of their work involves direct patient care. They work with patients to restore or improve mobility or assist patients with pain management issues. They may use techniques such as exercise, electrical stimulation, massage, hot and cold therapy, ultrasound or traction to achieve these results. Orthopedic procedures such as hip and knee replacements are becoming increasingly more common. Assistants will often provide rehabilitation services to these clients. Individuals involved in trauma-related accidents also require the services of physical therapy for recovery. Therapist sometimes rely on assistants to offer teaching related to exercises that can be performed at home or to deliver instructions on how to use crutches or rolling walkers. Physical therapist assistants are typically required to document and report patient responses to treatments and patient progress.

Most states in the United States require a assistant to hold an associate degree. In addition, some states require licensure. The American Physical Therapy Association reports that in 2009 there are currently 224 institutions supporting 237 accredited physical therapy assistant programs. There are also an additional 44 programs in development for this educational program. Completion of a state or national examination may be required for licensing as a physical therapy assistant as well.
